Birthday Package Request

Please complete this form to request a birthday package. Enter your name, preferred date and time, and specify the package details before submitting for approval.

Guidelines:

  1. Submit this to HRD a week before you intend to claim your birthday package with prior approval from F&B.
  2. May be availed on the birth month or within 30 days from employee’s birthday.
  3. To be presented to the Cashier upon arrival.
  4. Option 2 does not include incidental charges and is not applicable for ala carte meals, dine in only.
  5. Only regular employees are entitled for the birthday package benefit.
  6. Not valid on block-out dates and other F&B promotions. (e.g. Steak Nights, Seafood Nights, Christmas day, New Year’s Day, Festivals, etc.)
